Job description

Americare Kidney Institute is seeking advanced practice provider to work with our growing nephrology group. Provider preferred for nephrology clinics, will be very independent, but working in close collaboration with our physicians. Status: Part Time with potential for full time status. Hours: M-F day shift schedule. Location: Cleveland area primarily West- Side.

Job Responsibilities/ Essential functions:

Obtains complete medical history and physical data on patients

Establishes plan of care and implements interventions in accordance with nephrology protocols, and standards of care.

Order and perform routine laboratory/ diagnostic procedures

Monitor the effectiveness of therapeutic interventions.

Maintains and reviews patient records, charts, and other pertinent information.

Collaborates with other health care providers in an interdisciplinary approach to provide care to the patient and family.

Instructs patient and family regarding medications and treatment instructions. Provides patient education

Advances own professional development through self-directed learning, and continuing education.

Takes responsibility for self-development and supports a learning environment.

Initiates and maintains positive relationships with co-workers and leadership.

Displays commitment to Americare Kidney Institute and its core values.

Good written and interpersonal communication skills. Excellent analytical and decision-making skills.

Other duties as assigned



Requirements

Education/Expertise:

Current Ohio License

Basic Life Support (BLS) through American Heart Association (AHA)

Nurse Practitioner: Master of Science in Nursing with emphasis on clinical practice.

-or-

Physician Assistant: Completion of a formal Physician Assistant training from an accredited program with NCCPA certification.

Required Credentials, Licensure or Certification:

Nurse Practitioner: Current RN license to practice in Ohio Current APRN license to practice in Ohio Family Medicine or Adult/Gerontology Nurse Practitioner Certification

-or-

Physician Assistant: NCCPA-certified PA (PA-C) or eligible to be certified.

Must meet all requirements of the Ohio State Medical Board for Physician Assistants.

Preferred Qualifications:

New graduates welcome to apply. Experience in nephrology preferred.
