Position Overview
Gateway Wound Care (GWC) is seeking a compassionate, clinically-skilled Nurse Practitioner (NP) to provide advanced mobile wound care and primary care throughout the St. Louis metropolitan region.
This role offers the opportunity to practice at the top of your license while delivering evidence-based wound care directly to patients in skilled nursing facilities, assisted living communities, and private residences. The Nurse Practitioner will work collaboratively with an experienced supervising physician to improve healing outcomes, reduce preventable hospitalizations, and expand access to specialty wound care services.

What You'll Do
- Care for an established panel of patients through efficient, geographically optimized daily routes coordinated by the GWC team, averaging approximately six visits per day.
- Perform comprehensive wound assessments, evaluations, and ongoing management of acute and chronic wounds.
- Complete timely, accurate, and compliant electronic documentation for all patient encounters.
- Perform bedside procedures within scope of practice, including:
- Sharp and selective debridement
- Dressing selection and application
- Compression therapy management
- Offloading recommendations
- Negative Pressure Wound Therapy (NPWT) management
- Skin substitute and advanced wound product application (if credentialed/trained)
- Assess risk factors for delayed healing, hospitalization, limb loss, and other wound-related complications requiring escalation of care.
- Promote limb preservation, functional independence, and long-term health outcomes through proactive intervention and preventive care strategies.
- Conduct home-based primary care pertaining to chronic disease management, medication management, and routine follow-up (diabetes, CHF, COPD, and related conditions).
- Collaborate on individualized treatment plans with an experienced supervising physician.
- Partner with the GWC care coordination team to facilitate timely referrals to vascular surgery, podiatry, infectious disease, primary care, home health, and other specialties as needed.
Compensation
$107–$175 per visit, depending on visit type and clinical complexity. The upper end reflects more advanced, medically complex cases. Eligible for performance-based bonuses tied to quality metrics and patient outcomes.
Qualifications
Required
- Current Nurse Practitioner license in Missouri
- Current DEA registration (or ability to obtain)
- Valid driver’s license, reliable transportation, and familiarity with the St. Louis metropolitan region
- Strong clinical assessment, procedural, communication, and organizational skills
- Comfort working independently while collaborating with a supervising physician and interdisciplinary team
Preferred
- Missouri and Illinois licensure
- Wound care certification (WCC, CWS, CWCN, CWON, or similar)
- Experience with Medicare documentation and mobile healthcare delivery models
- Comfort working as a 1099 provider
Previous wound care experience is preferred but not required. Strong backgrounds in primary care, geriatrics, hospital medicine, home health, palliative care, vascular medicine, or post-acute care are encouraged.
